CDL/DOT DMV medical requirements for commercial drivers.
To drive in intrastate or interstate commerce, you must have passed a CDL medical examination, within the past 2 years, in accordance with Federal Motor Carrier Safety Regulations, as defined in the Federal Motor Carrier Safety Administration's (FMCSA) Medical Examination Report.
Acceptable proof of examination for the Motor Vehicles (DMV) is a fully completed Medical Examiners Certificate Form 649-F (6045). A medical examiner will need to complete the FMCSA Medical Examination Report for Commercial Driver Fitness Determination as well as the certificate at the end of the packet.
