 We will provide you with Form I-693, you don’t need to bring it with you.

 Special Instructions :

    The medical examination must be conducted by a physician who has been designated by USCIS as a civil surgeon. Doctor Ilene Z. Coopersmith is certified by USCIS as civil surgeon.
    Form I-693 has been revised in accordance with the Centers for Disease Control and Prevention (CDC) updates to the Technical Instructions for the Tuberculosis (TB) Component of the Civil Surgeon Medical Exam and the Technical Instructions for Vaccinations. 
  The revised Technical Instructions for Vaccinations include new age-specific vaccination requirements and became effective 07/01/08. The civil surgeon is required to complete the medical examination according to the Technical Instructions for the Medical Examination of Aliens in the United States, written by the Centers for Disease Control and Prevention (CDC). It is your responsibility to attend your medical exam appointment and all follow up exams and testing, as may be required by the civil surgeon. 
   After the medical examination is completed, the civil surgeon must give you the original and completed Form I-693 in a sealed envelop. Do not open the envelope.
  The civil surgeon must also give you a copy of the completed and signed Form I-693 and any supporting documentation submitted to USCIS to you for your records.
  The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status.
Updates: 

  1. Vaccinations: Starting on December 14, 2009, immunization against the Human Papillomavirus (HPV) and herpes zoster (zoster) will no longer be required. 74 Fed. Reg. 58634 (Nov. 13, 2009). Civil Surgeons who complete Form I-693 vaccination chart, Part 2.5 of Form I-693, should either:
    • write "no longer required" in the entries for Human Papillomavirus" and "Zoster," OR
    • leave these entries blank.
  2. Reminder: On or after January 1, 2010, the results of the medical examinations must be recorded on the October 14, 2009 edition of Form I-693. Any previous editions of Form I-693 that are submitted on or after January 1, 2010 will be rejected.
  3. HIV Testing: Starting January 4, 2010, an individual should no longer be tested for HIV infection for purposes of the immigration medical examination. 74 Fed. Reg. 56547 (Nov. 2, 2009). Please disregard any reference in Form I-693 and its instructions to HIV on or after January 4, 2010. When completing Forms I-693 on or after January 4, 2010, either:
    • write "No longer required" in the entry for HIV testing in Form I-693, section 2.2.(C); or
    • leave the HIV testing section in Form I-693, Part 2.2.(C) blank.

Note: The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status, but provide for sufficient time for the performance of laboratory testing or additional testing required under CDC’s Technical Instructions.
